When you fill out the staff tribute donation form, your chosen staff member will be informed of your appreciation and recognized with a special lapel pin. 

Who should I honour with my gift?

You can choose to honour any staff member who made a difference in your care – a doctor, a nurse, a case manager or any other member of our support staff.

Will my staff member be made aware of my gift?

Yes – in addition to being notified about your gift, your chosen staff member will receive a commemorative lapel pin, and your special message of thanks, if you choose to include one. The amount of your gift will not be disclosed.

How many staff members can I recognize and send a pin to?

You may recognize as many staff members as you like, however, each donation and thank you message must be made separately in order for each staff member to receive their lapel pin.
